A fatal chain-reaction collision occurred in Gravesend, Brooklyn when an FDNY ladder truck with lights and sirens activated ran a red light at the intersection of West 11th Street and Avenue S. The fire truck struck a white commercial van, then collided head-on with an Access-A-Ride vehicle carrying 78-year-old Placido D'Andrea and his wife, which subsequently hit a Lexus. D'Andrea was killed while his wife sustained injuries requiring hospitalization. Five firefighters and multiple vehicle drivers and passengers were also hospitalized in stable condition. The couple lived half a block away and were returning home from medical treatment. Their daughter discovered the accident at the scene. The incident remains under investigation with no arrests made.
