The FAA determined that the May 22 Starship Flight 12 launch resulted in a mishap involving the Super Heavy booster after stage separation. The booster failure occurred a few minutes into flight during the first launch of the upgraded V3 version. The booster separated from the ship but then appeared to suffer engine failure during the sustained burn intended to return it toward the launch site. The booster tumbled toward the Gulf and likely exploded on impact.
