Michael Chernus initially hesitated to lead Devil in Disguise because he did not want to glorify John Wayne Gacy or his "Killer Clown" persona. Creator Patrick Macmanus prioritized avoiding gratuitous violence, murders, and full depiction of the clown character. The series opens in 1978 with the search for missing 15-year-old Robert Piest and follows the investigation that uncovers 33 victims buried under Gacy's home. Each episode uses flashbacks to portray the lives of the boys and young men before meeting Gacy, emphasizing victims over lurid details. Chernus displayed "We Remembered" to honor unidentified victims and hope to restore names and faces.
