Nevada OSHA fined the company more than $112,000 in late 2023 after complaints of "ankle-deep" water in tunnels, muck spills, and burns. The company contested the violations. A construction worker suffered a "crush injury" after being pinned between two 4,000-foot pipes and required extraction with a crane. LVCVA leadership defended the project's regulation and said approvals would not have been granted if public safety standards were unmet. An LVCVA spokesperson declined to participate. Ben Leffel said repeated allegations indicate officials are not keeping the public safe and questioned whether a $250,000 penalty would be significant.
