A report by the Prisons and Probation Ombudsman revealed extensive falsification of welfare check records on suicidal inmates by prison staff. The report indicated a 35% increase in prisoner deaths, with 486 investigated in 2024/2025, including 100 self-inflicted cases. Additionally, complaints from prisoners have risen by 15%. The aging prison population and longer sentences contribute to these issues, alongside systemic problems with monitoring at-risk prisoners through Assessment, Care in Custody, and Teamwork (ACCT) protocols, which were undermined by falsified documentation.
The report from the Prisons and Probation Ombudsman (PPO) Adrian Usher found evidence that prison staff had lied over mandatory welfare checks for prisoners at risk of self-harm.
