A jury investigating the death of 20-year-old Jessica Powell determined that staff at the Summerlands Hospital failed to properly supervise and ensure the security of the therapy room, contributing to her accidental death.
Jane Yeandle, from the Somerset NHS Foundation Trust, expressed deep regret saying, 'We are very sorry that she died as a result of an incident in our care.' This underlines the seriousness of the operational failures in the ward.
Her father, John Powell, explained, 'It was just an error of judgement and unfortunately for Jess she just took her chance and that was the way it happened,' reflecting on the tragic circumstances leading to his daughter's death.
The inquest revealed significant lapses in safety protocols, including a faulty alarm system that misdirected staff during emergencies and inaccessible exits, indicating systemic issues in the hospital's operational practices.
